Il carcinoma a cellule transizionali della vescica è una neoplasia comune il cui trattamento medico non ha fatto significativi progressi negli ultimi venti anni. E’ una patologia complessa associata a elevate morbilità e mortalità se non trattata in modo ottimale. Per la malattia muscolo-invasiva, il “gold-standard” di trattamento è la cistectomia radicale. In casi selezionati alcuni pazienti possono avvantaggiarsi anche di una strategia multimodale chemio-radioterapica associata alla resezione transuretrale della neoplasia. Solo recentemente la chemioterapia per la malattia avanzata ha permesso di aumentare l’aspettativa di vita dei pazienti sia in prima che in seconda linea di trattamento, utilizzando schemi chemioterapici standard quali cisplatino/gemcitabina e vinflunina.
